The filling in the sleeping bag is supposed to share your body heat. If your clothing block your body heat from reaching the sleeping bag, the sleeping bag does NOTHING for you. Strip down! Boxer shorts is all you need. If you have cold feet from smoking and drinking you could buy those down filled socks.
And lastly, if you are cold or freezing before you get in your sleeping bag, you will stay cold for a long time. Maybe even the whole night. If you're cold, you won't have any heat to heat up your sleeping bag. Quick fix is to do some jumping jacks to raise your temperature before you go to sleep. Or chop/saw some firewood.
