I used a sanding drum on a Dremel at low speed and got it down as close as possible without going into the quick. You at least need to open up each section so the oil can get down into it. Then you have to apply the oil until that section grows out completely.
Creams don't work well on toenail problems because it is unable to soak into the nail properly even after grinding some nail off. Most people take an oral medication for toenail fungus in the nailbed, since nothing else will work if it has gone that far.
Prevention is better, keep dry, keep oiled, and inspect every day. Tea tree oil is very cheap for how many times you can use a small bottle. Use it before getting into wet situations and after showering when your feet are dry again.
