Kneepads. Hard hat with headlamp. Backpack full of water bottles and clif bars. Spare batteries. Large spool of bank line if you're in an unfamiliar cave without a guide. Bic lighter.

The bare minimum:

3 light sources
helmet
sufficient clothing. Most caves are going to be around 40 degrees F in summer time.
food and water.
Knowledge about white nose syndrome. If any of the caves you plan to visit are contaminated, remember to decontaminate all of your clothing and gear immediately after your outing.

No time to set up equipment or even get it there before he died. Even after 8 hours he was starting to go insane from being upside down. He was randomly passing out, waking up crying or laughing or screaming and hallucinating before passing out again. Eventually he woke up less often until he didn't wake up again.
