i was in a wheelchair most of my teen years, it can be done, but requires a lot more prep, research the trails you are going, and i am assuming this is just day hikes, because, although you can do it, overnighters are a damn hassle, i dont know whats wrong with the girl your dating, but if for whatever reason, she needs to get up, you will too, you sounds like a good guy so this might not be a problem, but just remember it
is she in a wheelchair? is it electronic? you mention that "she cant walk well" i am assuming this serverely limits her walking, like we are talking steps, not Km.
now i dont know this girl your dating, but, as a former and still kinda disabled person, being carried on someones back is way less then ideal, better the trip be shorter then signal to the whole world.
but, op, your preperation for this thing is what is most valuable, that you have even thought about this and are trying to work around her shortcomings to make sure she and you have a good time is important, without more details i cant really think of more to say
look for trains that are accesible by car
look for trails that have benches (if she can sit up by herself with no backsupport)
keep it short
take it slow
back when i was in a wheelchair the wort times were when i was no longer able to move or do something, like, on the toilet or something like that, technically i could do something in an emergency but flopping down on the floor isnt nice, so keep that in mind and work around it
