They don't make much difference in deep snow, but they're a godsend on glazed hardpack or ice, especially for black ice on the road. Riding around on the lake ice is a lot of fun too.
I've made my own with sheet metal screws and old mountain bike tires, back when commercial tires were either really expensive (Nokian) or really crappy (anything else). But it's a tedious pain in the ass, and now that there are more affordable commercial tires on the market I don't bother.
