Sure, it may not be as empty as the west in parts but it is still very remote in some areas and you are very far from a big city in much of central appalachia.
There's hundreds of miles of nice trails in the highlands of West Virginia alone (which doubtless you have never been to).
It isn't very populated at all in much of western maryland, central and northern/northwestern pa, or far south western virginia.
It isn't exactly empty but it is highly rural and there are plenty of secluded mountainous areas and wilderness type environments.
