They work best when used as close as you can get them to your skin without it touching. You need to be ideally using one of these before you are truly cold. They also are a moisture barrier and will facilitate hypothermia when improperly used.
In short, their use is like anything else. Skill and experience beats gear any day. Somebody that carries one of these and does not know it's limitations might be better off without it to "rely" on.
