The reel hangs under the rod, so you can use your index finger to hold the string when casting.
The reel over the rod is on those push-button kids rods that you cast with your thumb.

The issue with the pic is that he is holding it in his left hand when he should be holding it in his dominant right hand. You always hold the rod in your dominant hand.

How do you know he’s not a lefty? Tons of boomers switch hands when they are casting and reeling so they will cast righty then switch the rod to the left hand to reel righty as well.
