Vinegar or bleach.
Don't use oxidizing tablets like you might with plastic.

Be good if you could get a brush to scrub inside with.
At very least scrape the inside with something long and metal, tip it upside down and see if any flakes fall out.

i don't think you can do much to sanitize it... maybe steam it to death in a rice cooker or autoclave or the like.

i personally would never get comfortable with an old narsty cork like that. i'd replace it with a new'un, and keep the old one just for the sake of keeping the original pieces together
