Your story is one of those extreme cases where obviously you need some warm and well insulated clothing, and it resulted from a bad decision: so did mine, I thought I'd go faster if I didn't bring my coat, so I faced the Mongolian winter with a fleece only for a 10 minute walk to the next house. Things went well until the snowstorm, I got completely lost, and eventually sprained my ankle. And all the stuff I was wearing, save for wool socks, was synthetic.
Hypothermia kills in certain situations, and in those situations, what you're wearing hardly makes a difference. So yes, for most people in the world, hypothermia certainly isn't a risk, regardless of what they're wearing, and even in most cold places, cotton won't be a problem.
Not everyone lives in the same place as you do and does the same things you do. World is a big place, and your concerns are mostly laughable.
