Learned a few things using mine this summer:

Just basic stuff.
Ibuprofen, vetwrap, and athletic tape are probably my most-used items.
Cycle through your kit. If I slice my knuckle in the garage and need a bandaid, I'll take it out of my kit and restock it later. If the gauze packets are starting to look a little ratty and torn, throw it out and replace it.
A small piece of paper towel on top of the ibuprofen pills keeps them from rattling around and breaking, also helps keep them dry.
Trauma shears are nice to have, better(safer) than a knife for removing vetwrap/athletic tape.

I'm not a Dr. or EMT, and i'm not doing surgery innawoods. I'll bring enough for most common injuries, improvise when needed. I've used a motorcycle tube as a sling for a broken collarbone before. Stabilize the injury and go from there.

Bag fucking everything. Wet gear does you no good, ziplocs are cheap and effective.

Also, get your fucking first aid cert.
Best kit in the world won't do you much good if you don't know how to use it effectively.
Also for things like hypo-/hyperthermia, altitude sickness, shock, etc.

Yea.. the trauma sheers and tourniquet are a waste of weight for backpacking. More for people expecting to deal with major trauma all day every day (combat medics, ER nurses at level I trauma centers).

I'd put that weight towards extra tape, blister pads and saline solution....Multi-use things more likely to be needed and harder to improvise.

2nd this. And the ziplocs should be replaced every few months, they wear out and break their seams from friction inside the bag.

I use mini ziploc jewelry bags for pills. They're in the jewelry section of walmart or on amazon.

it's a useful tool, but definitely consider if one of your gear ties or compression straps could serve the same purpose.

I usually carry a very minimal set
-big roll of self stick elastic bandage. this is the most useful of all. supports joints, covers hot spots, compresses wounds and can be used day to day to bundle supplies or as cordage.
-ibuprofen
-acetaminophen
-imodium
-fine forceps
-nail clipper
-single edged razor
-band aids

that covers pretty much everything that I can reasonably expect to treat and still walk away from.

for irrigation, I use drinking water from a bottle with a squirt valve.

I also carry a PLB for truly life threatening emergencies.

Trauma shears weigh jack fucking shit and are really handy.
Saline is heavy and has like, 4 uses, only one of which can be used in the field without an iv kit. It's only really good for flushing wounds, which you can do with plain purified water.

This is the stuff thats immediately available on the outside of my pack that I use more frequently, or I can take off for just a day in the woods. If I don't bring a hatchet out it's the heaviest thing in my pack. But it could probably save my, or a buddies life so I deal with it. Outside of changing the bag there aren't many ways to reduce weight that don't involve removing items.

Sure, but if I know I'll be using an axe a lot I'd rather just have a tourniquet on me than having to fuck about with straps playing MacGyver when the clocks ticking. In some cases you even have to use two tourniquet to stop a bleeding.

Actually this is good planning.

Irrigation is better with a large volume of non-contaminated water than with a small volume of sterile water.

Saline is nice to have if a contact lens falls out and you need to wash it off. For wound irrigation I use about 10 times the bottle's volume refilling with potable water. Non chemically treated water, because the chemicals are irritants. Clear lake or stream water is fine.

"Proper" medical training tends to be geared toward certifications and licensing for occupations (paramedic, corpsman, physician, etc) and these are very reliant on technology. So those courses don't translate well to wilderness medicine. But courses that aren't for occupations (first aide, WFR, EMT) don't teach much more than how to apply bandaids.

Your best bet is hiring someone to teach private classes. You might spend more money but most of what you learn is relevant.

If you go to your local ambulance they may have ride along programs where you just go on 911 calls with them, but don't do anything. They can also direct you to EMT classes. In order to work or volunteer as an EMT yoy need to have taken and passed an EMT course, they do not provide that training on the job.

I took it during college actually and got certified. The course I did was 2 semesters (~9 months) and about 5-6 credits each. 3 hrs lab, 3 hrs lecture per week. Think it was open to the public for about $900.

Yes they teach more than bandaids, you just have to realize that means 30 seconds per topic. The material is a mile wide and 1 inch deep. I remember the obstetrics section...."This is a baby, this is the vagina it comes out of, any questions?" Next topic. Better than nothing I guess, and fun at times (the day learning to backboarding in the pool) but it's not what people take away from the course. You take away what they hammer into your brain every day...."Scene safety, BSI".... ABCs, clearing everyone from the AED, backboarding, suctioning, etc....Not much of that is useful in wilderness medicine. But it does bring back some nostalgia.
