Unless I had the insect dead in a light box, I don't think I could use a tripod. There's no way I could get what I get already. Keep in mind I'm using e.tubes and sometimes those + reverse lens. Like in this image and it is a freehand pic. So this setup is just under 6.5 inches long, not including the body. This fly....it would not hold still at all. Not that it matters since this is a extreme as this setup will go. The DOF is that tiny band across the fly's eyes.
For less magnification the DOF is wider and things get much easier for freehand.
