The patch cable needs to have a male tip-ring-sleeve (TRS) on the mic side and a male tip-ring-ring-sleeve (TRRS) into the phone. A TRS on both ends won't work because the jack on your phone includes an input for a microphone. If you use a mic with a standard male TRS attached, like a Rode VideoMic Pro, buy a female TRS to male TRRS adapter.
If you're more ambitious, run a few cameras and synchronize in post. It will be more interesting than having a single hard camera and you won't have to fool with resetting. Just be sure to do a single clap right after you're rolling so software can find the synchronization point in your footage's audio tracks.
