If you get a decent macro lens and light it well you can get shot like this. The Canon MP-e 65 or Laowa 2.5-5x would be great for this. You could also put tubes on other high end macro lenses and probably be fine. Laowa makes very interesting macro lenses. I have their 15mm macro. It's not L glass but for the price and uniqueness of it I am quite happy with it.
There are plenty of small ring and twin-lite flashes that mount to the front of a lens that will work just fine. Since they are close to the subject they do not need to be particularly strong. Keep in mind though that light from these creates a very typical shot. If you use them people who have been around the block will look at your photo at know. It can also make for a generic image but if you hit the right technical level and niche who the fuck cares.
I have the canon 100L macro, MP-e 65, and Laowa 15mm if you have any detailed questions about the process. I also have some manual bellows, auto tubes, reverse mount, and in the past has the non-L version of the 100 macro. I've never really cared for reverse mounting, tubes, and bellows.
