It's very possible it comes from the scanning process, the Epson film holders are far from ideal, but I've never bothered to try anything else to keep it perfectly flat, it might be as simple as that.
The scanner also might be out of focus, it's old and it's travelled a bit, it probably needs to be re-calibrated
