Every Nikon TC with AF (back to 1992), and every Canon EF TC (1988) has protruding elements. Sony didn't with the A-mount, but did with the E-mount. Olympus didn't with the original 4/3 mount, but they do with m43 (along with Panasonic). Fuji does as well.
Pentax, and 3rd parties, are really the only one that don't nowadays. You might have better compatibility with using lenses you otherwise couldn't (honestly probably shouldn't bother), but that comes at a decrease in optical quality. Even Pentax knew this, which is why the old "pro" quality TC's from Pentax used to protrude, and they made cheaper / more compatible TC's that didn't.
