I double checked my notes and you need the front plate mount off an old/cheap extension tube for the lens to fit into and a reverse ring or rear plate of an extension tube of the other mount type that goes into the camera body. Here's a screen of my notes. It even shows what to use if you want to add a rear lens filter to them. Just use stackable filters for that.
