To get whole bug in focus, many different images where focus varies are used and combined together in post processing (usually automatically). This ranges from only two to up to a hundred images. Subject must be completely still for the whole process. There must be no wind. In most casees it's either dead, frozen, or was motionless for long enough period. In some cases whole process can be handheld, but usually it's on fixed tripod. It's somewhat complicated process, but it can be done with nearly every camera that allows lens changes.
