E.g. 6 blade aperture will create hexagonal bokeh.
In mirror lenses it's a donut, which gets reflected by a mirror donut on the inside, reflected again by the small circle mirror on the front element, and then enters the camera.
You can get custom bokeh shapes by cutting them out of opaque material and using as a filter in front of the lens. For example popular shape is heart bokeh.
