(1) When you've got a girl posing, one of the most elementary things that really helps with, well, everything, is making sure they elongate the neck and pull their chin forward.
That type of posture doesn't come naturally for anybody unless you've been a professional model for years. So as the photographer, you've got to direct them to do it. But man, it's huge. It'll really help out her posture, her neck, and all the smushed skin on her face. Just simply tell her to bring her chin forward a bit and it'll be huge.
Again: She won't do it herself. It feels unnatural. You've got to direct her to do it.
(2) You've got to learn some basic skin editing techniques. Her chin looks like a fucking ball sack here. This is a quick and dirty edit (since I'm working with a low res jpeg), but I think this will give you a few ideas of things you can do with just a little D&B. I used the 50% grey overlay method here because it's quick and easy for basic stuff.. There are plenty of different methods out there. Experiment a bit and find one that works. My favorite is using masked curves layers.
This isn't a polished edit. Just to give you an idea of what she'd look like without a testicle chin.
