There’s no detail advantage in 1:1 shots which is what you might traditionally want medium format for, it seems better on the A7rIV due to the colour moire on the X1D, then you could use pixel shift for landscapes if that’s really not enough still.
ISO performance seems a little better on try X1D, there’s one f/1.9 lens but you may not want that focal length for every shot, you might be stuck with f/4 or f/3.5 which then pushes the ISO advantage back to the A7rIV since you’ll be able to use a lower ISO since you’ll have a faster lens, plus the X1D doesn’t have IBIS, so you when you take that into account the difference is even bigger.
There is no real world IQ advantage here for the X1D. It might be good for prestige, as a status symbol, for high paying clients. Perception is everything when it comes to money.
