It’s not so much the camera (I’ll guess some kind of half frame contraption) as it is letting the print fade for 40 years before scanning it.

It's mainly the age and fading that you're probably asking about, but the square format and age suggest 126-format film, which was mainly used in the Kodak Instamatic series of cameras.
