The east is very poor, and just like it's already been said here, the desperation of poverty is usually what drives them into it. More sinister things will keep them there.
Speaking as a photographer currently living in the United States, I know it happens here too but not nearly like it does in other places. Most western models you'll run into are just people who got noticed and groomed since adolescence to be able to handle themselves on camera. Most the time they're making at least as much as I do, and live in a society that values them higher than war heroes.
