If her eyes don't look very sharp, it's gotta be for some other reason. Most likely, simply because this particular copy of the shot is fairly low resolution, so there aren't enough pixels making up the eye for you to tell if they're in sharp focus or not so you're just assuming they're not so you can have something to bitch about.
Also, given some context clues:
1. 210mm f/8 in the filename
2. Aspect ratio closer to 4:5 or 4:3 than 3:2
3. Looks like film
he was probably shooting with either medium format or large format. Either way, 210 is not going to be a super long telephoto (and might even be ~50mm equivalent, if it's LF), the camera's gonna be pretty large, and the subject of the shot is standing still right in front of him looking straight into the camera.
I.e., it's not a creep shot. It's a posed portrait.
