It will also happen if you're holding the camera close enough to your body when it's cold out (say you put your cold camera inside your jacket).
If you want to avoid it, keep your camera warm. If your lens fogs up after coming indoors, just let your camera warm up before trying to shoot. You could speed the process up by blowing warm, dry air over the lens.
Putting your camera in a ziploc bag could work, but only if it's actually air tight and only if the air in the bag is dry. You would still have to wait for your camera to warm up before taking it out.
