What causes this shape of bokeh?

I remember seeing it in a movie, where the bokeh was roughly this shape but even closer to a rectangle, and on its side. Is it because of barn door lens shades being too tight or something?

The shape is primarily determined by the aperture blades.

Obviously a shaped aperture will get this, I'm just asking if theres a reason why movies sometimes have it. I can guarantee you million dollar productions arent using a plastic aperture insert.

ANYTHING to get the look you want, even if it means a homemade jerry-rigged lighting fixture or using velvet in place of gels in front of lamps

Spherical zoom lenses tended to only use six or eight blades. Blade design wasn't perfected yet, so it would at some occasions net op or triangular specular highlights.

I don't know a concrete case but I know they would do it if needed. What I told you about Tovoli is a lot more unorthodox.

You're spewing bullshit. You don't know of a case since it was never used. Inserts fuck up light gathering, good luck calculating exposure using them. Every noob that ever fooled with them, will know their weakness. Spec highlights shape as described by op comes purely from aperture design.

odd number of aperture blades and blade shape usually produce what you're describing. polygonal aperture with an odd number of blades like 5-7. most famous lens goes up to 9. cooke, angenieux and zeiss were all creating these types of lenses in the 70s. there were a lot of them earlier, but a different shaped star burst / bokeh and flaring became popular in the 70s. earlier lenses had a lot of blades at an even number, which created a very round bokeh.

most new cine lenes are 11-19, which have fairly round bokeh when shot wide open, but when you stop them down, they produce different shapes.
