For example I'm sure Punk understands how fucked up it is to tell someone with severe mental issues to chop their dick off so they can be pretty princess. And since he's so well informed on political and social issues, he might even know that John Hopkins (which pioneered the surgery in the US) stopped performing it years ago because the suicide rates were sky high; meaning that it doesn't work and its basically a form of life long torture. But he does it anyways for The Message, he might even think its funny.
