They seperate far away from the bone and aren't predisposed to grow that much, the actual bone width isn't narrow, a narrow frame for a man means the shoulders aren't wider than the waist. It's just that his delts and trap insertions are far from the bone and don't push outward. Same as when someone has bicep insertions far from their elbow, they will never get the bicep to go all the way to the forearm when flexing.
