Shawn's "back injury" is one of the biggest works of all time, he wasn't wrestling because he was a unreliable drug addict and Vince didn't need him anymore. If he wasn't on pills he could've been back by WrestleMania 15 at the latest.

Had Shawn been able to do Wrestlemania X7 and Stone Cold not turn heel at X7, Triple H probably would have beaten Undertaker at X7.
Triple H beat Austin clean, for seemly, no reason in their 3 stages of hell match. It was likely going to lead to a feud post Mania, but since Austin turned heel, they teamed instead.
Shawn would have probably returned at X7, interfered to help Triple H, and Triple H would have beaten Undertaker.

It wasn't a work, he was thinking of making a comeback throughout 98, he was trying to rehab his back without surgery, but eventually he realised he needed to have surgery in early 99ish. After the surgery he just gave up and got high all the time. He could've been back by 2000 probably but he was too busy being depressed and getting high or whatever lol. His surgery was reported in local newspapers

Even assuming his '98 back injury was legit, he was totally fine by this point and wrestling some shows for his school. Every week in the Observer there was another paragraph about his comeback but it just never materialized because he was an unhinged druggie they couldn't rely on to turn up sober or make shows at all. Of course WWE would have wanted him to put Rock over too and that wasn't happening.
