She often bumps wrong (spreading her arms out to early or too late) and hits the back of her head against the mat. The 619 gives such an ample window of time for the receiver to put their hands up and protect their face. Riho isn't at fault at all. The fault falls squarely on that wrestler and the people who trained her.
