Lex got her hooked on pills and caused her death.
It's why he's basically become persona non grata since her death, as just about everyone in the business, but especially those who knew Elizabeth personally, despises Lex and blame him for her dying.
The only two who didn't were ironically Savage (who had pretty much moved on by that point and washed his hands of Elizabeth) and Sting, the later of which blamed himself because he told his wife that Lex was cheating on his wife while on the road as part of him becoming a born again Christian in order to save his marriage and she told Lex's wife (who she was friends with), breaking up their marriage and leading to Lex hooking up with Elizabeth and getting her hooked on pills. Leading to Sting blaming himself for setting into motion Lex hooking up with Elizabeth and her death.
