Once someone's off the list it becomes very hard to find a dentist who is able to take them on without going private.
A sad fact of most poor people is that they're poor because they have zero discipline, so they're the ones who end up never bothering to go to checkups because "nothing's wrong with my teeth so it's a waste of time" and then they lose their dentist and can't afford a private one.
Also, a lot of the tooth stuff is overblown - Brits have considerably healthier teeth than Americans on average, but Americans are very appearance oriented so they do the whole tooth whitening bollocks and get a false impression of what healthy teeth look like. The tooth might be rotting away on the inside but they don't care as long as the tooth looks white.
