Once you've built a significant amount of muscle and trained your CNS, you don't need to keep blasting "juice" for the rest of your life. You can keep 70-80% of those gains - which for most is plenty - just by keeping up your conditioning, eating a shitload of protein and lifting heavy. If you get injured, you're fucked and it will be a much longer climb back up top. But once you're there, most of the grinding is done.
John Cena likely used some very illegal substances early in his career and at various other points (injury/illness recovery, etc). But the amount of people just using and using and using, every single week for 20+ years straight, they drop dead early for a reason. Those substances are HELL on the liver/kidneys - and if you add the stress of red-eye flights, getting little to no sleep, backstage fights/politics, unhealthy road food, relationships falling apart, etc, it's enough to give you heart issues WITHOUT any drinking, smoking or PED abuse piled on top.
