Once odds get too big, say even from 1/1000, or a 1000-sided die, you can roll those odds for an infinity and there is a chance that you will never ever land on that one side of the die. Games need a worked RNG to ensure you can actually roll things with bigger odds, like if something is 1/256, they'll usually have it automatically have to let you get it within 256 tries then reset, or else you could technically roll forever. Either that or they fix it so it that 1/256 is randomly set, like this time you'll get it on the 43rd try, next on the 112th. There are instances in games, like FFX-2 where you have to search for an item in the desert, and they didn't work the RNG so some people spend countless hours looking for the thing and some people claim they just never got it.
