Not saying I believe all or even most of the Meng stories, but there are plenty of times when we get a call of a disturbance or fight, and by the time we get there the persons involved have already left and the other persons remaining are unable or unwilling to identify them or say what happened (it's possible that various local police departments have reports of Meng-involved incidents, but no one actually identified Meng in those incidents to the police at the time)
Then the persons involved refuse to come to the police station afterwards to make an official complaint (usually because the idiot victim who got assaulted wants to "handle it himself" thereby getting himself in further problems, then after they go get their revenge the other person flips the script and presses charges on them, turning the victim into a suspect)
As for the medical records part, people can go to the hospital for treatment with the good old "I fell" excuse, or they just simply refuse to tell medical staff who assaulted them when it's a clear assault case
The doctor may or may not call and inform us (they usually only do it if it involves a minor victim, a woman in a suspected domestic violence case, or anyone with gunshot/stab wounds or major maiming/mutilation). After that, the most we can do is visit the patient and press them for details but if they're an adult that clams up, then there's not much else we can do except document our investigative efforts
