That was never a rule until they wanted to do it in matches and it magically became a rule. If two guys are outside the ring then there is one count, by your own logic if someone is outside the other person has to go outside at the exact same time or else there are two counts or the count is broken, but it isn't. If you go out and the other guy goes in then the count stays and if you stay outside for some reason then you're counted out. It's simple. The worst offender is tagging in other people so you can all be in the ring at the same time, if this was a thing then why don't tag teams just keep all members perpetually tagged in and fight at the same time?

It's up to the ref's discretion, in cases where it happened in the past he gives the guy a final warning and the guy hops in the ring, there was one match I can't remember with who but the guy on the outside throws the guy coming out into the ringpost and jumps back in so the other guy is counted out.
Refs can also call off the count if one guy is seen to be trying to prevent the other from getting back in the ring.
In NWA and other feds you had to be inside on the other side of the ring or the ref wouldn't count out the other guy.
