The likeliest scenario is that they are extremely depressed and think constantly about how they look and how other people perceive them.
If they strike first and say someone who is objectively attractive is "ugly" or point out one of their flaws etc.-- in their mind they are placed ahead of them in the social ladder.
It helps when you're anonymous and nobody knows how fucking disgustingly ugly you are irl.
This same things goes for people who attack those who are more successful/have achieved something they dream of.
It sounds childish but it really is just jealousy.
