I'm pretty sure this is real. I have seen other photos/videos of folks with this much or more scalp exposed due to infected flesh. The thickness of the scalp (swelling) and less blood than your average head-wound (scar tissue) don't seem completely implausible. You can CGI in flies but the way they stick in his hair and crawl around means they're real. You can put some stinky stuff on your head to draw flies sure, but seems unlikely for a trick. A longer video posted below has a narrator speaking in spanish, saying that the man is cutting away infected scalp. Nerve endings will die in very infected areas, so while this would be unbearably painful for a fresh wound, the gradual drug-induced picking away at it might not be painful at all or minimally so for this guy. There are no prosthetic seams that I can see. The hair is real. The coloration is consistent. I see literally no evidence pointing to this being fake other than folks wanting to believe it is because holy shit is this awful.

This is 100% real.

It was filmed in Tijuana Mexico in one of the higher poverty Zones around Downtown, its called Zona Norte, the dude just removed his scalp because of drugs and mental issues, no local hospitals wanted to take him, they had to transfer him to Mexicali, Mexico where he got some medical attention.
