He wanted Otis to wear trunks because he thought he looked like a fat guy in a speedo and it was funny so he made sure at the very least he wouldn't wear the straps over his shoulder when he wore the singlet. He wanted him to shave because he has a lot of fat under his chin and Vince thought it made him look comical but in a good way. Same thing with the worm.
