Khali had a match with a guy who died awkwardly. Not really his fault. He performed a flapjack or some kind of move where the guy comes down face first, and he fucking died somehow.

The Sasaki thing is that a trainee definitely died in training under his watch, but there were reports that he immediately tried to save him, etc. And it's important to note that Sasaki didn't stop being a trainer after this, which means it probably wasn't ruled his fault. He did end up training Tanahashi for example.

The rumors that Sasaki killed the kid came later when a former dojo mate of Kento Miyahara came out and said that Sasaki and Nakajima used to roughly haze new trainees, so people conflated that with the accident previously and that's how rumors are born.
