Everyone gets one roll per turn. They can decide to spend it as they wish: "invade xyz," "take uncontested territory in x," "give half my roll to y (at least if you allow it)," "defend if I'm attacked," "if x happens (like filling a territory) then do y." Some players have more complicated instructions but that's the gist.

Player B won't necessarily automatically defend himself (he might be focused on some other priority, have rolled already and not checked the thread to update on what to do with his roll instead of his initial plan) in which case Player A can get that territory uncontested. If they attack each other it's just who rolled higher. Player A rolled a 6709 and Player B rolled a 5475.
9 is 18, 5 is 12. Player A gets 6 new territories that Player B previously controlled. If Player B had bonuses like control of Texas they would have canceled each other out. If Player A had them he'd get additional territories. Let's say Player B instead rolled a 5575, that's 30 in which case it's down to his instructions and how you interpret them. "Counter attack" or "attack is straightforward he just invades Player A instead. I've seen "defend" turn into the player just holding onto orginal territory with the rest going to other objectives or being wasted (iirc that caused arguments sometimes; some Risk OPs find it easier to just do the attack each other thing).
