Usually I try to conserve threads a bit so a single thread contains multiple playthroughs. The setting is urban occult sort of like Tom Clancy Rainbow Six tactical guns with Tarot magic lol. The 1d78 roll means to draw a Tarot card with replacement from the initial 78 card Tarot table in the first paragraph.
The first and initial playthrough is more narrative driven. The feel of it is familiar to those who know Vampire: Masquerade, Mage Ascension and also films like Collateral, The Neon Demon by Nicolas Winding Refn and also Don DeLillo's novel Cosmopolis. It is deliberately meant to be disorientating at the start... maybe too much so, as some anons have mentioned.
The subsequent playthroughs are more dungeon crawl / OSR with some randomly generated NPCs or scenario elements as well. Because some of the anons played through my older quests (entirely diceless quests) there are some continuing factions from my other settings, but they are not too important to these playthroughs.
I recognise that my quests are difficult and I do kill / fail / bad ending the quests when I think the players have no idea what is going on. Perhaps this is a bit unfair (I am not infallible, I make mistakes lol). It is just that I cannot steer the quests towards a good happy ending given some of the players conflicting actions (maybe a better QM can do this...)
