After a solid five minutes of being slapped by Jack to finally be awakened, the old man is eventually lead to the anomaly's containment cell. Jack quickly leaves before anything happens.
You make a simple announcement in the anomaly's cell. "Shot, do whatever you want to him." He's going to die anyways, right?

D-US: Where am I? Who are you?
A tendril <span class="mu-r">pierces right through the man's head.</span>
ANOMALY: Do not resist. You must be saved. I can save you.
Despite that, the D-Class is still standing. Even with a great big nothing in his head.
D-US's hands shake while he tries scribbling down some notes.
D-US: Who are you? Why am I here? Huh? Huh? Write...?
The anomaly's constant pulsing stops.
The tendril that pierced right through the man's head tilts to the side.
ANOMALY: The hell? Why are you taking this so well? The other recruits screamed.
ANOMALY: Perhaps you are one of the few good ones in this excuse of a facility.
D-US: Taking what well?
A chunk of D-US's head falls to the floor. His left arm looks pretty raw.
The tendril sniffs at the converting D-Class.
D-US keeps scribbling even as his fingers nearly rot off.

The two are in a loop of sniffing and scribbling.
D-US's leg is starting to melt away, revealing bone.
ANOMALY: Impressive...
Observing each other. Seeing who will blink first.
D-US blinks. His posture suddenly stiffens as the anomaly's conversion is completed.
D-US's left arm raises, then his right.
D-US kicks out his left right, then his right.
ANOMALY: Conversion is completed. Welcome to the army, soldier.
D-MEAT: Huh. My head feels clear now. I...huh. The last few hours were a blur.
D-MEAT: I thought I was in Gamma-Company still.
The meatball squirms but otherwise does not respond or say anything else.
ANOMALY: I think you should stand with the rest of the army, soldier.
ANOMALY: At ease. Welcome to the club.
D-US exits the containment cell.
D-US, now a meat man, lingers around the rest of the army. Yet there's a sparkle of light in his eye that isn't in the others. He flexes his fingers before leaving the horde, notes in hand.

Probably less of a penalty, if any, than him dying. You're content with this.
