Though I figured out how to extend Karn’s prime without (probably) disastrous consequences. He just has to do similar thing Mechikabura did. Mechi wished to regain his youth with help of Dark Shenron. Karn has Divine Dragon Force so he could do something similar. I mean the result would be Karn young again not “eternally young”. Now I get that he may not want do it, but he is one of few people who can do this if he ever changes his mind. Same goes for some debilitating illnesses, the “Wish” can fix that.

With his youth restored he could fight at his best level and if he was defeated it would be because opponent was stronger and not because “Bad guy only won because Karn was old. Karn in his prime would win this fight for sure.”
However if Karn does not want to do this, I understand.
