They should be aware that common sense is rare among the players and make things clear enough, so that quest can run smoothly without extra confusion or false assumptions. Sometimes you just have to give MC witcher senses, hint at someone being a potential ally or enemy. Especially if players get the wrong impressions, that could potentially ruin the narrative you really wanted to pursue.
In addition to that, QMs should control the flow of action to some extent, so the players don't get lost in a sandbox or open 5 different paths and then leave them, because the "qst-ADHD" or they are forced to move on. Hell, tie the paths together and make them end in a main-quest related hook at the end if that will stop the chaos. It's not a bad thing to abandon some arbitrary realism in order to make the game more enjoyable.
With all that gay shit out of the way, I would like to say that RQM does a phenomenal job at introducing and presenting things/characters we encountered in the past. His short descriptions make it so that if you're new or simply forget something or someone, you will always roughly know what's going on and have a chance to understand and bond with a character.
Many a QM just state names expecting the same level of knowledge and familiarity as they have from the players, making so that you gotta ctrl+F and have no investment in NPCs.
