I think it was the time my players basically wrote in the idea that the QMC's backstory dead sister was still alive (based on some write ins about his memories of how exactly she'd gone out thirty or more years ago) - and I came back with her now being a Duchess Of Hell because she'd taken a portal straight there and started berserking around. You keep what you kill, and you are what you eat. And the way she went to Hell, she technically wasn't dead.
The sister was planned to be nothing more than a backstory character for a couple of flashbacks, but some write-ins got me typin', and she became a recurring character eventually. A divisive one, but I liked her for the most part.
