There isn't much knowledge on the higher structure of the combine overall. There are the Advisors, which seem to at least partially have their own personalities, and seem to have general authority over Breen and dictate actions done on Earth.
But, the Combine are made of many species, and with the fact that many species are incorporated biologically into the Combine, the Advisors are more likely just a local authority of a greater overmind. Cooperated by the differently level of cannon lore implicating that it was The Combine which assimilated the Advisors, instead of the Advisors being the ones that started The Combine.
The striders, the gunships and dropships are all Synths, species like humans that were assimilated into the Combine. So if one discounts humans transformed by The Combine, then no, there are no "true" Combine on Earth. Closest we get are the advisors.
