One needs to enter "gaimv2" and then answer the various multiple choice questions (suggest having a secondary tab with Google translate). At the end of the survey there's an optional field for comments about products you'd like to see (ie. I said I would like the stage shows on blu-ray and would by all Gaim, Drive, and Super Sentai SHFs).
