Quill's dad actually made it into 616 before he did and wore a version of it before Giffen retconned Quill into 616. Quill wore it again in Bendis' Avengers Assemble, Age of Ultron, as well as Deadpool vs Thanos, often minus the helmet (I really wish the figure had an alternate head). And finally there was a version from the future that hung out with the original GOTG in Abnett's Guardians 3000. That's pretty much it I believe.
