They most likely have a mental illness, some brain deficiency and chemical imbalance that causes a person to feel like shit with very little impact of what goes on around them.
Like sure they would most likely still rather sleep in a comfy bed and have water to drink but ultimately feeling "good" is a commodity that some will never experience because they are simply broken on a biological level.
